New Sub Contractor Application

Requirements to be added to our sub contractor workforce


  1. GL Certificate - General Liability Policy listing K.R. Siding & Gutter as "ADDITIONAL INSURED".
  2. WC Certificate - Workers Compensation Policy. We do not accept DOL issued waivers.
  3. Current IRS W9. Click here for fillable template.
  4. Contractor license for the work you will preform. For siding you must have a B100, R100, or S230 State issued contractor license.
  5. Completed sub contractor application. Please DO NOT fill out the application unless you have submitted all forms.
  6. Once all documents are received and approved, a Master Service Agreement or MSA must be completed before any PO can be issued.
